Practice Policies

KJL For Life, Inc. · Kimberly Lazarus, LCSW

This website is not monitored 24 hours a day, 7 days a week. If you are experiencing a mental health or medical emergency, please call 911 or go to your nearest emergency room. You may also contact the 988 Suicide & Crisis Lifeline by calling or texting 988, available 24/7.

Appointments & Scheduling

Sessions are scheduled in advance and typically occur on a weekly or biweekly basis, depending on your individualized treatment plan. Please arrive on time; sessions cannot be extended to make up for a late arrival.

Cancellation & No-Show Policy

I require at least 24 hours' notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice, and missed appointments without notice, may be billed at the full session rate. Insurance does not cover missed or late-cancelled sessions.

Fees & Payment

Payment is due at the time of service unless other arrangements have been made in advance. I accept the insurance plans listed on the Insurance page; for out-of-network or self-pay clients, a receipt (superbill) can be provided upon request for reimbursement purposes.

Communication Between Sessions

Email, text, and phone messages are for scheduling purposes only and are not monitored continuously. Please allow up to 1–2 business days for a response. These channels should never be used for urgent or emergency situations — see the notice above.

Confidentiality

What you share in therapy is confidential and protected by law, with a few important exceptions required by Illinois and federal law, including: suspected abuse or neglect of a child, elder, or dependent adult; a serious and imminent threat of harm to yourself or another identifiable person; and situations where disclosure of records is compelled by court order. These exceptions are discussed in more detail in the HIPAA Notice of Privacy Practices.

Telehealth

If sessions are conducted via telehealth, you are responsible for finding a private, confidential location for each session. Telehealth may not be appropriate for every clinical situation, and this will be discussed together as needed.

Termination of Services

You may end therapy at any time. When possible, I ask that we schedule a final session together to review progress and discuss referrals, if needed. I reserve the right to refer a client to another provider if I determine I am not the best fit to meet their clinical needs.
